Prospero Health and Social are seeking a dedicated Nurse Practitioner with experience in aged care to join a dynamic healthcare team in Hunter Valley, New South Wales. About the Role:
As a Nurse Practitioner, you’ll play a vital role in delivering exceptional care to elderly residents. Your responsibilities will include:
- Collaborating with Multidisciplinary Teams: Work closely with GPs, geriatricians, and other healthcare professionals to manage patient care and treatment plans.
- Geriatric Care and Advanced Care Planning: Lead advanced care planning discussions and provide expert advice on the care of residents, including those in palliative care.
- Clinical Leadership: Review new treatments, technologies, and therapeutic interventions to ensure the highest standards of care.
- Patient Education: Provide educational resources for residents, families, and carers, supporting informed decision-making and empowering individuals in their care.
- Record Keeping: Maintain clear, concise, and accurate resident records, ensuring compliance with healthcare standards.
- Continuous Professional Development: Participate in ongoing education and mentorship activities to further develop your clinical expertise.
What We’re Looking For:
To thrive in this role, you’ll need:
- Qualifications: Registered Nurse with postgraduate qualifications as a Nurse Practitioner.
- APHRA Endorsement: Must hold or be eligible to apply for endorsement with the Australian Health Practitioner Regulation Agency (APHRA).
- Experience: A minimum of 5 years as a Registered Nurse, with at least 3 years of full-time experience in aged or community care settings.
- Clinical Expertise: Strong knowledge of the Nurse Practitioner Standards of Practice as set by the Nursing and Midwifery Board of Australia.
- Patient-Focused Approach: A passion for patient care and a collaborative, team-oriented mindset.
- Excellent Communication: Strong interpersonal skills and the ability to engage with patients, families, and healthcare teams compassionately and effectively.
- Adaptability: Willingness to embrace the challenges and rewards of working in a busy, fulfilling aged care environment.
