Nurse Practitioner – Aged Care | Sydney, New South Wales
Prospero Health and Social is actively seeking an experienced Nurse Practitioner with a strong background in Aged Care to join a reputable care home in Sydney, New South Wales.

Role Overview:
As a Nurse Practitioner, you will play a pivotal role in delivering exceptional care to residents, collaborating with Geriatricians, GPs, and a dedicated healthcare team. Your responsibilities will include:
- Consulting with Geriatricians and GPs on diagnostic orders and treatment plans.
- Leading Advance Care Planning for residents.
- Providing expert guidance in palliative care.
- Contributing to the adoption of new clinical practices, treatments, and technologies.
- Maintaining accurate and concise resident records.
- Delivering professional development activities and educational resources for residents, their families, and carers.
What We’re Looking For:
- Registered Nurse with postgraduate qualifications as a Nurse Practitioner.
- Eligible for endorsement with the Australian Health Practitioners Regulation Authority (APHRA).
- A minimum of 5 years’ experience as a Registered Nurse, with at least 3 years in aged or community care.
- Solid understanding of Nurse Practitioner Standards of Practice (Nursing and Midwifery Board of Australia).
- Strong commitment to patient-centred care and the ability to collaborate effectively within a multidisciplinary team.
- Excellent communication skills and a compassionate, proactive approach to patient care.
- Willingness to work in a dynamic and fulfilling aged care environment.
